If yes, please provide more information below...Required Purchases Every student is required to purchase a Music Diary and Music Pouch ($12.00, available from the Purchasing Office) and a Tutor Book, which will be ordered by the Instrumental Teacher through the school and you will then be notified of its arrival. Terms & Conditions* You must read and agree to each of the following terms and conditions. Discontinue Notification *Once lessons have commenced, we require a minimum of five teaching weeks' notice to discontinue. I agree to give five teaching weeks' notice to discontinue.Absentee Notification *If a student is absent for a lesson due to sickness, you must notify the relevant Instrumental Teacher directly by 8:00am the morning of the lesson. Calling the School alone will not suffice as it is highly unlikely the Instrumental teacher will receive the message before 8:00am. Students who have their lesson at 8:00am or 8:30am need to notify at least one hour before the lesson time. Any other absences require a minimum of one week notification. Failure to do any of the above will result in a charge for the lesson. I agree to notify the Instrumental Teacher of an absence as outlined herein.Second Instrument *If your child wishes to learn a second instrument, this should be out of class time and must be discussed with their Classroom/Homegroup Teacher, as well as the Instrumental Program Coordinator. I agree to discuss with my child's Classroom/Homegroup teacher and the the Instrumental Program Coordinator as outlined herein, OR my child does not wish to learn a second instrument. Responsibility for Year 6+ *Once students are in Year 6, they are responsible for arriving at their lesson on their own. Students will not be collected from their class.
